Adult Clubs
Your individual Club affiliation forms will be distributed Clubs in May/June 08. These are pre-populated with data supplied from 2007-08 to minimise the time required to complete them. Please make sure you highlight any changes or errors.
The completed forms should be returned to the Manchester FA Office by 30 June 2008 in order to avoid a late affiliation fine being applied.
Youth Clubs
Youth Club affiliation forms will be distributed direct to Clubs in May/June 08. Please ensure the Teams List is fully completed so that every team is recorded. E.g. U10 A, B, C etc.
NOTE - The FA has been promoting the appointment of welfare officers in clubs and leagues with youth teams for the past couple of years. This requirement in now mandatory and failure to appoint will result in affiliation not being accepted. New Clubs
New Clubs should download the appropriate blank affiliation form from "Related Documents" and once complete return it to the Manchester FA Office. Please note that new clubs need to contact Leagues in the county directly for respective affiliation.

Please remember that separate affiliation forms are required for:
Adult Clubs
Youth Clubs/Teams
Please use the 'Extra Team Form' opposite if you need to affiliate more teams.
