Please note all CUP ENTRY forms are required to be returned to the office by 30th June 2010.
Adult Clubs
Your individual Club affiliation forms will be distributed in early May 2010. These forms are pre-populated with mandatoray information supplied from 2009-10 to minimise the time required to complete them.
Please make sure you highlight any changes or errors.
The completed forms should be returned to the Manchester FA Office by 30th June 2010 in order to avoid a late affiliation fine being applied.
Youth Clubs
Please ensure you read the guidance notes on the right before completing your affiliation form. Youth Club affiliation forms will be distributed direct to Clubs in May 2010.
NOTE - For clubs who provide youth provision (u18's) it is now mandatory to have a Club Welfare Officer and failure to appoint will result in affiliation not being accepted. For clubs with 10 or more teams, TWO Club Welfare Officers are required.

If you wish to affiliate to Manchester FA please contact the Business Admin Unit on 0161 604 7620 option 2.
Please note that new clubs need to contact Leagues in the county directly for respective affiliation.
